We described herein a patient who presented an overlap syndrome of childhood-onset systemic polyarteritis nodosa (c-PAN) and childhood-onset systemic lupus erythematosus (c-SLE). A 9-year-old girl presented tender subcutaneous nodules on feet, arterial hypertension, right hemiplegia and dysarthric speech. She was hospitalized due to stroke and left foot drop. Brain computer tomography showed ischemic stroke. Magnetic resonance angiography revealed stenosis in the middle cerebral and internal carotid arteries. Electroneuromyography identified a mononeuropathy of left posterior tibial nerve and she fulfilled the c-PAN validated criteria. She was treated with intravenous methylprednisolone pulse therapy followed by prednisone, that was progressively tapered, six months of intravenous cyclophosphamide and after that she received azathioprine for 19 months. At the age of 14 years and 9 months, she presented malar rash, photosensitivity, edema in lower limbs and arterial hypertension. The proteinuria was 1.7g/day. Antinuclear antibodies (ANA) were 1/1280 (homogeneous nuclear pattern) and anti-dsDNA antibodies were positive. Renal biopsy showed focal proliferative and membranous glomerulonephritis. Therefore, she fulfilled the American College of Rheumatology classification criteria for SLE and she was treated with prednisone, hydroxychloroquine and mycophenolate mofetil. In conclusion, we described herein a possible overlap syndrome of two autoimmune diseases, where c-PAN occurred five years before the c-SLE diagnosis.

The interaction between Authentication, Authorization and Accounting (AAA) systems and the Quality of Service (QoS) infrastructure is to become a must in the near future. This interaction will allow rich control and management of both users and networks. DIAMETER and DiffServ are likely to turn into the future standards in AAA and QoS systems, but they are not designed to interact with each other. To face this, we propose a new Diameter-Diffserv interaction model and describe the Application Specific Module (ASM) implemented to ...

Future telecommunication networks, and in particular networks beyond 3rd Generation, will be based on an “all-IP” architecture. Issues such as Quality of Service (QoS), Mobility and Authentication, Authorisation, Accounting and Charging (AAAC), and their respective interoperation, must be handled in such scenarios. This paper describes a generic end-to-end QoS architecture and its relationship with the other networking components. This architecture is being developed inside the IST project Moby Dick.

Current trends in communication networks point to an aggregation of all kind of traffic (data, voice, etc) in the same transport technology. IP is on the lead to be that transport technology and this is valid both for fixed and wireless networks. The usage of IP as a transport technology solves some of the interworking problems between different technologies, but at the same time, the IP Quality of Service (QoS) mechanisms are not fully adapted to the reality of QoS-aware mobility between different technologies. This poses ...

This paper describes a global end-to-end Quality of Service architecture for Beyond-3G networks. This “All-IP” architecture supports multiple access networks technologies (WCDMA, WLAN and Ethernet), and interoperates with Mobility and Authentication, Authorization, Accounting and Charging (AAAC) systems. The different network entities involved in the overall QoS delivery process are introduced. Additionally, scenarios and signaling mechanisms for providing peruser and per-service QoS as well ...

The increasing utilisation of QoS mechanisms creates many problems to commercial data networks. The multiplicity of requirements posed requires new and scalable approaches from the network operator. This paper discusses a highly flexible, scala ble architecture for processing QoS Admission Control in public networks. The architecture relies on the cooperation of two different entities, an agent and a manager, with fully distributed implementation, and able to perform the required signalling, admission control, and realizing optimised admission control decisions. This architecture is capable of operating with multiple QoS frameworks, with minimal added overhead.

This paper presents a next generation network demonstrator currently under deployment in Aveiro. The demonstrator supports mobile IP with fast handovers with integrated QoS and AAAC issues considerations. A monitoring system provides network information adequate for QoS management, and a specially built QoS Broker is able to manage multiple types of Access Routers. The demonstrator has been tested with some applications, and is currently under refinement.
